Reimbursement rates in NEMT vary widely because they depend on multiple factors: the payer type (Medicaid, VA, workers’ comp, private insurance, or private pay), the mode of transport (ambulatory, wheelchair, or stretcher), the distance traveled, the time of day, and the level of assistance required. Medicaid trips often follow a state-set fee schedule, which can be lower as compared to contracted rates and private-pay rates.

Instead of one flat number, providers should think in terms of a base fee plus mileage and various ancillary charges. For example, a set rate for a wheelchair pickup plus a per-mile charge could average $35 - $60 base pus $3 - $5 per mile, while stretcher trips can be significantly 3 – 4 times higher. Ancillary charges can include deadhead mileage, after hour and weekend fees, wait fees, bariatric fees, and more.

Profitability in NEMT depends on how quickly you secure steady trip volume and manage your expenses. Some providers begin covering their costs within the first 6 - 12 months, especially if they start lean with one vehicle and build private-pay or facility contracts early. Others may take longer if they rely heavily on Medicaid brokers or face high startup costs for vehicles, licensing, and insurance.

The reality is that there’s no fixed timeline - your payer mix, marketing, efficiency, and ability to control overhead all play major roles.
